COVID Passports

We kindly ask you to refrain from calling us for help with your COVID Passport as we need to keep our lines available for those who are unwell.

Please call 119 or use the NHS APP to obtain your Covid Passport as we are unable to provide this for you. 119 are incorrectly signposting patients to their GP for support with the NHS App and Covid Passports.

If you wish to have access to your medical record where you will be able to see your vaccination status including the booster, please request this through our website, through the Administration Office.
